Eligibility Criteria & Requirements

Age Requirement: Adults 18 years and older with intellectual or developmental disabilities
Documentation: Medical records, psychological evaluations, and diagnosis documentation
Funding Source: Medicaid waiver, private pay, or other approved funding sources
Iowa Residency: Must be eligible for Iowa DHS services or willing to establish residency
Care Needs: Assessment confirms we can safely meet the individual's care requirements
