Eligibility Criteria & Requirements
Age Requirement: Adults 18 years and older with intellectual or developmental disabilities
Documentation: Medical records, psychological evaluations, and diagnosis documentation
Funding Source: Medicaid waiver, private pay, or other approved funding sources
Iowa Residency: Must be eligible for Iowa DHS services or willing to establish residency
Care Needs: Assessment confirms we can safely meet the individual's care requirements

family members, healthcare providers, and our interdisciplinary team. These plans address
medical needs, behavioral support, personal preferences, goals, and daily routines. Plans
are reviewed and updated regularly to reflect changing needs.
Absolutely! We encourage family involvement and visitation. Families can visit during
reasonable hours, and we welcome participation in care planning meetings, activities, and
special events. We believe family connections are vital to residents' well-being.
We accept Medicaid HCBS waivers, private pay, and other approved funding sources. Our
admissions team can help you understand available funding options and navigate the application
process for Medicaid waivers if needed.
Our staffing ratios vary based on residents' needs and time of day, but we maintain ratios
that ensure safety and quality care. We exceed state minimum requirements and provide 24/7
staffing. Higher-need residents receive more intensive one-on-one support as needed.
All staff are trained in emergency response procedures and certified in CPR and First Aid.
We have emergency protocols in place and maintain close communication with healthcare providers.
In case of emergency, we contact emergency services immediately and notify family members or
guardians as soon as possible.
Yes! Community integration is an important part of our program. Residents participate in
regular community outings, shopping trips, entertainment events, and volunteer activities.
Family members can also take residents on outings with advance notice.
We maintain high quality standards through ongoing staff training, regular supervision,
